Help At Home Services
Skilled Nursing
Our Skilled Nursing services provide professional, in-home medical support to help older adults stay safe, healthy, and independent. Licensed nurses offer personalized care including health monitoring, medication support, wound care, wellness checks, and coordination with healthcare providers — all delivered with compassion in the comfort of home.
Homecare
Our Homecare services provide practical, everyday support to help older adults remain safe, comfortable, and independent at home. Caregivers assist with tasks such as meal preparation, grocery shopping, essential housekeeping, and laundry — helping make daily life easier while supporting independence and peace of mind.
Personal Care
Our Personal Care services provide respectful, hands-on support with daily activities to help older adults maintain comfort, dignity, and independence at home. Caregivers assist with bathing, grooming, dressing, mobility, exercise, oral hygiene, skin care, and other personal needs — always delivered with compassion and professionalism.

Plan of Care
At The Hub, we believe the best care begins by seeing the whole person. That’s why every Help At Home plan of care is thoughtfully personalized — built around each individual’s needs, goals, and what helps them feel safe, comfortable, and independent at home.
The process begins with a home visit from our Access Care Coordinator, who takes time to understand each person’s situation and strengths. Together — and often with family members or loved ones — we create a customized plan of care that outlines services, cost sharing, and the right level of support. As needs change, the plan is reviewed and adjusted to ensure care continues to fit the individual’s life.
Help At Home is a licensed Home Health Agency and may accept payment through the Medicaid Community Choices Waiver, the Veteran’s Administration, private pay, and long-term care insurance.

Professional Staff
Help At Home services are coordinated by an Access Care Coordinator certified through the Wyoming Department of Health – Aging Division, ensuring each person receives personalized support designed to help them remain safely at home.
Personal care is supervised by a Registered Nurse and provided by Wyoming State Board of Nursing–certified Home Health Aides (HHAs). Our team receives extensive training in home health and aging-related care, delivering compassionate, professional assistance that supports dignity, independence, and peace of mind. All staff are thoroughly background checked before entering any client’s home.
Community partnerships help make this care possible. Support from local businesses — including Fremont Toyota of Sheridan and Let ’Er Buck Car Wash — helps our aides travel safely and reliably to serve older adults throughout the community.
Hours of Operation
Our team is here to serve you Monday through Friday, 8:00 AM–4:00 PM, excluding major holidays. We look forward to helping you find the support you need.
