Navigating benefits and community resources can be overwhelming. The Hub on Smith’s Outreach Case Management team is here to help connect individuals and families with programs and services that support health, stability, and independence.
Many of these resources are available to people of all ages—not just older adults.
We can assist with:
- Low Income Energy Assistance
- Medicare Part D and Supplemental Insurance (Ages 65+)
- Property Tax Refund Applications
- Low Income Housing Resources
- Homeless Veterans Assistance
- Referrals to additional local, state, and federal support programs
